12 MONTH DELUXE OIL BURNER SERVICE CONTRACT

Must Be on Automatic Delivery

To our customers 1 complete service contract
At a yearly cost of $395.00

Cleaning of Boiler or Furnace Flues	Cleaning of Water Level Tube(Steam)
Cleaning of Smoke Pipe	Checking Lock Out Safety Control
Function & Adjusting of Limit Controls	Checking Proper Water Pressure(Boilers)
Replacement of Nozzle	Replacement of Line Filter Cartridge

Checking Proper Temperature Setting for Fan Control (Furnace)

 Regular service hours under contract are covered (Mon. – Fri. except Holidays)
Winter: Oct. 1 – April 1: 8:00 AM – 4:30 PM. Summer: April 1 – Oct 1: 8:00 AM – 4:30 PM
All service not covered by contract will be charged at prevailing rates in effect at
that time. Any labor performed after Regular Hours (Holidays, Weekends) will be
charged as normal working hours.

The parts listed below are covered for replacement or repair under this contract:

Fan Limit Control Burner Blower Cage
Nozzle Delayed Oil Valve
Nozzle Line Circular Relay
Pressure Limit Control (Steam) Gaskets
Fuel Pump Cad Cell Eye
Ignition Transformer Cad Cell Relay
Electrodes & Porcelains

Expressly EXCLUDED from this contract are the following:

1. Failure of low water cut off (steam system).
2. Combustion chamber low water cut off, water feeder circulator pump, blower motor, blower fan, blower bearings, zone control equipment, air filters, smokepipe, draft regulator and wiring.
3. Heat distribution systems, hot water or steam piping and radiators, ductwork systems, bleeding and flushing systems.
4. Humidifiers, air cleaners.
5. Oil floods.
6. Lack of oil.
7. Failure of customer to maintain proper water level.
8. Defective or plugged oil lines.
9. Tank sediment and sludge problems.
10. Air conditioning portions of combination heating and cooling controls.
11. Any valves or components pertaining to water end of system.
12. Sludge related problems due to dirty fuel oil tank. (Clogged oil lines, filter, nozzle, etc.)
13. Water in fuel tank.
14. Frozen oil lines and/or jelled fuel.

General Terms

1. John Guire Oil reserves the right, and is the sole judge in determining the acceptance of equipment under this policy.
2. Automatic renewal effective only upon receipt of payment.
3. Policy void if all oil needs are not purchased from John Guire Oil Co.
4. Policy void upon closing or transfer of account name, address, or location.
5. Right is reserved to decline acceptance of service agreement until after inspection.
6. This plan is only available to owners purchasing their heating oil directly from John Guire Oil.
7. Service under this plan does not include labor or parts made necessary by fire, flood, or other abnormal conditions.
8. Low water cut off from steam systems must be flushed regularly by home owners to prevent sediment. We assume no responsibility for their failure as they are not covered by service plans.
9. Acceptance of service plan does not obligate us for responsibility for secondary damages, freeze-ups, tank leakage, or other conditions resulting from delays or failure to render service or oil delivery due to conditions beyond our control.
10. This agreement will remain in force for one year from date of acceptance and will be self-renewal for each succeeding year at our regular established rates.
11. Not responsible for damage done to vacant structures due to heating system failure including being out of oil.

This plan may be cancelled upon 30 days written notice by either party.

Prices and terms are subject to change on the renewal anniversary date of this agreement without prior notice.

No refunds or transfers of these service plans.

All service plans subject to cancellation if not paid in 30 days.
